What We Do

Brighter Days Network provides Support Coordination for adults enrolled with the New Jersey Division of Developmental Disabilities. Our work centers on person-centered planning, clear communication, and consistent follow-through.

Step Process

6-Step Support Coordination Process

1. Welcome

We begin with a warm introduction to learn about the person’s strengths, needs, routines, preferences, and goals.

2. ISP Development

We create or update the Individualized Service Plan to reflect the person’s goals, needs, and supports.

3. PCPT Completion

We complete the Person-Centered Planning Tool to document key details about daily routines, communication, health, safety, preferences, and strengths.

4. Service Connections

We connect individuals to DDD-approved providers, employment services, day programs, transportation, CBSS, clinical supports, and community resources.

5. Ongoing Monitoring & Updates

We maintain regular communication, update plans, respond to changes, and ensure supports remain aligned with the person’s evolving needs.

6. Annual Reviews

We complete the annual review of the PCPT and ISP to reflect progress and any new goals or support needs.

What is Support Coordination?

Support Coordination is the process of helping individuals with developmental disabilities and their families navigate the resources available to them. From medical services to social, educational, and community-based programs, our role is to ensure that all necessary support is provided efficiently and effectively. As a released agency authorized by the Division of Developmental Disabilities (DDD), we are able to fast-track service approvals, ensuring that care begins without delay.
