Location: New Jersey (Statewide – Field-Based / Hybrid)
Position Type: Full-Time and Part-Time
Position Overview
The Support Coordinator plays a critical role in supporting adults with developmental disabilities by coordinating services, advocating for individual needs, and ensuring that supports are delivered in a person-centered, compliant, and outcome-focused manner. This role requires flexibility, strong time management, statewide travel, cultural responsiveness, and adherence to all certification requirements.
Responsibilities
- Complete Person-Centered Planning Tools (PCPT) and develop Individualized Service Plans (ISP) in accordance with DDD and SCA requirements
- Coordinate, monitor, and document DDD-funded services and natural supports
- Complete NJCAT reviews and ensure accurate service tier alignment
- Maintain timely, accurate, and audit-ready documentation
- Conduct required monitoring, follow-ups, and service verification
- Travel throughout New Jersey’s 21 counties to conduct face-to-face meetings and required visits
- Advocate for individuals’ rights, preferences, and access to services
- Collaborate with providers, families, guardians, and community partners statewide
- Identify risks or barriers and ensure appropriate follow-up actions
- Comply with all policies, timelines, regulatory standards, and certification requirements
Qualifications
Required:
- Bachelor’s degree in Human Services, Social Work, Psychology, or a related field
- Active Support Coordinator Certificate issued by The Boggs Center on Developmental Disabilities
- Minimum of one (1) year experience working with adults with developmental disabilities
- Knowledge of New Jersey DDD systems and services
- Willingness and ability to travel extensively across all 21 counties in New Jersey
- Strong written communication and organizational skills
- Valid driver’s license and reliable transportation
- Ability to pass background checks and required clearances
Highly Desirable
- Bilingual in Spanish or another language
- Prior Support Coordination experience in New Jersey
- Familiarity with iRecord and NJCAT systems
- Experience maintaining audit-ready documentation
